Quovus Pty Ltd is committed to protecting the privacy of participants, families, referrers and staff. This policy explains how we handle personal and health information.
Information we collect
We collect personal and health information necessary to deliver behaviour support, assessment, training and consultation services. This may include contact details, NDIS details, clinical and diagnostic reports, behavioural history, incident information and records of restrictive practices.
We collect information directly from participants, their guardians or nominees, referrers, support teams and other treating professionals with appropriate consent.
How we use information
Information is used to assess referrals, conduct assessments, develop and review behaviour support plans, deliver training, provide clinical consultation and meet our reporting obligations as a registered NDIS provider.
Disclosure
We disclose information only where it is necessary for service delivery, where consent has been provided, or where disclosure is required or authorised by law. This may include disclosure to the NDIS Quality and Safeguards Commission where required.
Storage and security
Records are stored securely within our document management system, with access limited to authorised personnel. We take reasonable steps to protect information from misuse, loss, unauthorised access, modification or disclosure.
Access and correction
You may request access to, or correction of, personal information we hold about you. Requests can be made by emailing admin@quovus.com.au or calling 0431 764 455.
Complaints
If you have a concern about how your information has been handled, contact us at admin@quovus.com.au. We will acknowledge your complaint and respond as promptly as possible.
